A Member may change their beneficiary at any time. The Member is required to complete the Pension Plan Appointment or Change of Beneficiary Card. A sample of the current "Pension Plan Appointment or Change of Beneficiary Card"is shown to the right of this section. This card is available from your employer, or administrative agent.
The following information must be identified on all change of beneficiary cards and returned to the administrative agent for processing.

- Member information
- Name
- Social insurance number
- Date of Birth
- Gender
- Beneficiary's information
- Name
- Social insurance number
- Date of Birth
- Relationship
- Current date, with your signature (front and back) along with that of a Witness (front)
